Gordillo-Escobar et al. speculate that the age of blood and storage lesion could bias the results of our trial. Transfusion practice in patients with septic shock was not based on the age of blood at the TRISS trial sites. These factors may be important, and data from ongoing randomized trials will help us to understand whether the age of blood affects the outcome of critically ill patients (Standard Issue Transfusion versus Fresher Red Blood Cell Use in Intensive Care [TRANSFUSE] trial [ClinicalTrials.gov number, NCT01638416] and the Age of Blood Evaluation [ABLE] trial [Current Controlled Trials number, ISRCTN44878718]). The observed loss of FEV1 was neither a time-dependent nor a dose-dependent effect in the withdrawal group, as compared with the maintenance group. A difference of 38 ml between groups became apparent only after the final step of inhaled glucocorticoid withdrawal and did not change to a meaningful extent thereafter. Singanayagam et al. ...

Budesonide for Kids Reduces Adult Height

Adding tiotropium to standard combination therapy may help reduce exacerbations in some adults whose asthma is poorly controlled despite the use of inhaled glucocorticoids and long-acting beta-agonists, according to two randomized, controlled trials published in the New England Journal of Medicine. However, tiotropium use did not increase the number of symptom-free days or boost patients’ asthm...

DESIGN Systematic review of randomised controlled trials of children and adults with asthma comparing the addition of anti-leukotrienes or placebo to inhaled glucocorticoids. Does roflumilast decrease exacerbations in severe COPD patients not controlled by inhaled combination therapy? the REACT study protocol

BACKGROUND Many patients with chronic obstructive pulmonary disease (COPD) continue to suffer exacerbations, even when treated with maximum recommended therapy (eg, inhaled combinations of long-acting β2-agonist and high dose inhaled corticosteroids, with or without a long-acting anticholinergic [long-acting muscarinic antagonist]). Roflumilast is approved to treat severe COPD in patients with ...

Inhaled steroids in exacerbations of COPD.

I read with interest the recent article by GUNEN et al. [1], wherein the authors presented the hypothesis that nebulised budesonide might be an alternative to systemic corticosteroids in the treatment of patients with exacerbations of chronic obstructive pulmonary disease (COPD). There are certain points which need discussing regarding their hypothesis of nebulised inhaled corticosteroids in CO...
